Safeguarding Adults Review: Rose- Report and Action plan (April 2019)

Rose, a 75-year-old woman, relied on a wheelchair and required assistance at home, where a hoist was used to aid her mobility. When her husband, her primary caregiver, had to undergo hospital treatment, Rose was temporarily admitted to a care home for respite. However, just two days into her stay, she began experiencing shoulder pain, and later, bruising was observed on both her breasts and arms. Concerned about her well-being, her family decided to bring her back home after only four days in the care home.

Tragically, that evening, Rose started choking on her food, prompting immediate hospitalization. Her condition rapidly deteriorated, leading to her eventual passing.
