Safeguarding Adults Review- Mr F (June 2020)

Mr F was a 38-year-old man with a diagnosis of Spina Bifida who lived independently in his own adapted accommodation. Mr F had a history of severe leg ulcers, urinary tract infections and was self-catheterised. He was not followed up and was closed to all agencies apart from his GP for a period of 5 years. Mr F was supported by his father on a weekly basis. He was found by his father unresponsive with two large swellings on his foot and knee which were infected and rotting. The Coroner recorded a cause of death as septicaemia and pyelonephritis associated with a urinary tract infection and infected leg ulcer
